The Opiates
History
Excavations of the remains of neolithic settlements in Switzerland (the Cortaillod culture, 32002600 B.C.), have shown that Papaver was already being cultivated then; perhaps for the food value in the seeds (45% oil), which we know as poppy seeds. The slightly narcotic property of this plant was undoubtedly already known then. NOTE 11
The milky fluid extracted from the plant's ovary is highly narcotic after drying. This is then opium. The writings of Theophrastus (3rd century B.C.) are the first known written source mentioning opium. The word opium derives from the Greek word for juice of a plant, after all, opium is prepared from the juice of Papaver somniferum.
The Arabic doctors were well aware of the beneficial effects of opium and Arabic traders introduced it to the Far East. In Europe it was reintroduced by Paracelsus (14931541) and in 1680 the English doctor Sydenham could write:
'Among the remedies which it has pleased Almighty God to give to man to relieve his sufferings, none is so universal and so efficacious as opium.'
In the eighteenth century opium smoking was popular in the Far East and the opium trade was a very important source of income for the colonial rulers the English, the Dutch, with even the Spanish getting their share in the Philippines. Although opium was readily available in Europe at that time, its use was not problematical.
Opium contains a considerable number of different substances, and in the nineteenth century these were isolated. In 1806 Friedrich Serturner was the first to extract one of these substances in its pure form. He called morphine after Morpheus, the Greek god of sleep. Codeine (Robiquet, 1832) and papaverine (Merck, 1848) followed. These pure substances supplanted the use of raw opium for medical purposes. Like opium they were frequently used as painkillers and against diarrhea. The invention of the hypodermic in the midnineteenth century lead to widespread use of morphine intravenously as a painkiller.
In the United States opiate use rose greatly in the last century, partly because of the opiumsmoking Chinese immigrants, and partly because many of those wounded in the Civil War were given it intravenously. In addition many 'patent medicines' contained opium extract: laudanum, paregoric, etc. It was partly due to this that morphine also became fashionable as a 'remedy' for opium addiction; for if the doctor gave an opium addict morphine, he was no longer interested in opium so he was cured.
This was also the case in Europe and although its use was at that time much more widespread than is now regarded as acceptable for medical purposes, it led to few problems. NOTE 12
At the end of the last century, the United States started to try to curb the nonmedical use of opium, especially in China, and later tried to prohibit it. American interest here was twofold: they wanted an economically strong China as a market for their own products, and the moral element played a major role. As a result of the SpanishAmerican War, the Philippines became American and the new rulers were confronted with a widespread problem.The American bishop of the Philippines, Charles Henry Brent, carried on a moral crusade in the US against the opium trade and opium addiction, and found widespread support. And not only because he was riding on the waves of Prohibition, for as we have already seen, unlike the European countries, the US also had a domestic opium problem.
China, with its economy weakening, also saw the rise of a strong antiopium movement. England and the Netherlands, however, looked upon this development with disfavor as the cultivation of the papaver was a very important source of income for Britain and Dutch East India.
In 1909, under American pressure, representatives from countries with colonial possessions in the Far East and Persia met at Shanghai to hold the International Opium Conference, chaired by Bishop Brent. This conference laid the foundation for the International Opium Conference in The Hague in 1911. The English proposed that for participation in this second conference and the treaty that would result from it the condition be set that the effects of the treaty should extend to the preparation and trade in cocaine and morphine. The Germans had considerable difficulty with this condition as their pharmaceutical industry substantial interests in this area.
The conference lead to the first international convention, the Opium Convention of 23 January 1912, although it went no further than obliging the affiliated countries to take measures to control the trade in opium within their own national legal systems. The Germans were eventually successful in having the wording changed in all articles to do with morphine and cocaine from 'undertake to' to 'try to'. The ratification of the convention was ultimately made dependent on countries not present at the conference, in short it was as leaky as a sieve.
A second conference, held in The Hague in 1913, was equally unsuccessful in effectuating the convention and it was only at the third conference in The Hague in 1914 that a protocol was signed allowing the convention to take effect without the signatures of all the participating countries.
The United States immediately gave substance to this convention with the Harrison Narcotics Act of 17 December 1914 which not only controled the trade, but went much further by making illegal possession of substances named in the convention by unauthorised persons. A maximum fine of $2000 and/or five years imprisonment was the penalty set. The basis for the criminalization of the use of drugs had now been formalized!
World War I brought all efforts to a standstill, and the matter only came up again after the Treaty of Versailles was signed. In this convention the US introduced the provision that all countries which had not signed and/or ratified the convention of 1912 should still do this. The convention was handed over to the League of Nations in 1920 for enforcement.
In England the Dangerous Drugs Act came into force in 1920. Of interest here is that while the Americans also outlawed the use of heroin for medical purposes, the English upheld this usage and even found the provision of opiates, in this case heroin, to addicts to be acceptable medical practice.
As stated earlier, the treaty of 1912 was 'as leaky as a sieve' because it allowed the states to determine for themselves when and how they would fulfil their obligations with regard to opium, which of course kept the use of opium legal until that time. The chemical derivatives did, however, fall under thisĀ· commitment: their use was illegal, making these substances more than opium the object of the battle. To make this battle more effective the League of Nations held two conferences which led to two Geneva Conventions: one of 11 February and one on 19 February 1925.
The first convention concerned limiting the domestic production of and trade in opium in the colonies in the Far East. The second extended the number of substances covered under the Convention to include the coca leaf, raw cocaine, ecgonine and Indian hennep. Also, the states were to step up monitoring of the preparation, trade and possession of the 'numbing' substances involved.
Use as such was not made a punishable offence. After all, opium was still being legally cultivated and consumed in the East. An opium monopoly was seen as an effective way of combatting misuse.
In 1931 there was an international change of course and efforts were also made to forbid the legal production and consumption of opium for nonmedical purposes. New conventions were signed for this purpose: the conventions of Geneva (13 July 1931), of Bangkok (27 November 1931) and Geneva (26 June 1936) a more and more complicated network of conventions. The last convention especially for the suppression of the illicit traffic in narcotics went further towards criminalizing the use of drugs by requiring the convention partners to lay down harsher punishment, in this case with imprisonment for all offenders of the provision from the relevant conventions.
Ironically enough the Americans did not sign this one because it did not go far enough.
After World War II the United Nations took over the matter. The Economic and Social Council of this organization set up the U.N. Commission of Narcotic Drugs. This Commission, made up then of 40 member states, started preparations for a worldwide drugs policy.
This resulted in the Single Convention (New York, 30 March 1961) which replaced all previous conventions with one.
Under this convention all parties are required to take the necessary legal and administrative measures to restrict the trade, production and possession of narcotics to scientific and medical purposes. All activities which are not directed towards these scientific and medical purposes must be considered as punishable offenses.
The convention has four lists of substances with regard of which a different regime of supervision applies, and on recommendation of the World Health Organization (WHO) the UN can add certain new substances to these lists. However it must be shown that these substances present a serious threat for public health or are involved in illicit traffic. The first is a clear criterium, the second clearly not. As long as a substance is not forbidden, production, trade and use can, of course, not be illegal!
Depending on the degree of misuse, substances from one list can be put on another. National legislation would then have to be adapted to these changes.
It is of interest with this to note when the European ratified all these conventions drug abuse was not a social problem. Unlike all other laws, the opium laws in Europe were not introduced as a reaction to a social problem, but were more or less imposed by foreign countries, namely the United States, the '...barbarians of the West' for their 'extraordinary savage idea of stamping out all people who happen to disagree ... with their social theories' against narcotics, against alcohol and in 'their recent treatment of Socialists'. NOTE 13
And, the world was a victim of American puritanism, for in Europe it was really only still in a few Chinese communities that nonmedical opium was used. It was no longer a problem in Asia either now that the aggressive sales tactics by the colonial rulers had ended. That is also disputed in most European countries, but in the Netherlands, in Amsterdam and in Rotterdam, it was tolerated as long as its use remained limited to the Chinese.

The pharmacology of opiates.
The pharmacological effects of opiates result from the fact that these substances have a bit (like a key) just like the endorphins and thus directly stimulate the endorphinreceptors (fig. 9). Because the opiates were known earlier than the endorphins, these are usually called opiatereceptors. We can trace these receptors in the brain by injecting radioactive opiates and then by monitoring where the radioactivity collects in the brain. This appears to be in very specific areas in the brain. Figure 10 shows these areas in diagram.
The first concentration of opiatereceptors is formed by a nerve cell system which plays an important role in transmitting pain stimuli. A brief digression regarding pain is required here.
If someone unexpectedly pricks herself, for instance on an improperly stored needle in the sewing box, she will already retract the injured finger (and bleeding or not, put it in her mouth) before any pain is felt. This is due to an emergency telegraph from the finger to the spinal cord from whence another message is immediately transmitted back to the arm muscles (comparable to the kneejerk reflex). At the same time, a message from the spinal cord is transmitted to the cortex of the cerebrum, which results in the first experience of pain. Until then, there are only signals aimed at a direct reaction to end the painful stimuli. If that were to be the end of it, there is every chance that the person would put her hand into the sewing box just as carelessly on a second occasion. In order to prevent this, and to introduce a moment of learning, stimuli are sent (slowly) from the spinal cord to the part of the brainstem where the opiate receptors are located.
This area is responsible for the alarming or threatening aspect of pain and it is exactly this effect which is remedied so effectively by the administration of opiates. The feeling itself does not disappear so much as lose its threatening character. It is this which lends the opiates their painkilling (analgesic) effect.
The most striking quality of this painkilling effect of opiates is that it has virtually no effect whatever on the other sensory perceptions, consciousness or the motor functions. All other substances with a painkilling effect, such as laughing gas, alcohol, ether and barbiturates also have, in an effective dose, a definate effect on consciousness, motor coordination, the intellect and emotional control. The drowsiness which can be caused by opiates is experienced only at high dosage.
A concentration of opiate receptors are also located in the respiratory center. These cells serve as a kind of metronome, that apparatus countless people have standing on their pianos to keep the beat. This metronome regulates the breath in a similar way, with fast or slow settings according to requirements, but allowing in and out breaths to take place regularly. Opiates also have an inhibiting effect on these cells: both the frequency and the depth of breathing is reduced under the influence of opiates. In the case of an overdose, respiration can come to a complete halt. Through shortage of oxygen, the heart muscles can no longer beat and as a result, brain cells die, and death occurs. Besides this, opiates inhibit sensitivity to the impulse to cough. Codeine in particular is used in many cough remedies, but even heroin is used for this purpose in England.
The third concentration is in the vomitting center, which, stimulated by the stomach (contaminated food etc.), normally causes the stomach muscles to contract, resulting in vomitting. These cells are stimulated into activity by opiates: opiate use causes nausea nd vomitting. However, tolerance for this effect is built up very quickly, although some users continue to vomit after each 'shot' for years. This effect is strongest with the opiate apomorphine, which is used medically specifically for this purpose.
The effect of opiates on the digestive system, which also contains large numbers of opiate receptors, has been known about for the longest period of time. Long before opiates were used as painkillers, opium was used for diarrea: opiates inhibit intestinal peristalsis. For this reason, most heroin addicts are constipated.
Opiates also affect the endocrinal system. By influencing the hypothalmus, the part of the brain linked to the hypophysis, the conductor of the hormonal orchestra, body temperature is slightly lowered, although it goes up with chronic use of high doses. Via the hypophysis, opiates lower the amounts of cortisol and testosterone in the blood, although these effects disappear again with chronic use as a result of tolerance.
Opiates influence the pupils: they contract (miosis). This is an extremely reliable signal of opiate use. Besides this, when suffocation occurs (as a result of respiratory inhibition) in the case of an overdose, the pupils dilate (mydriasis).
In the usual therapeutic dosage, morphine widens the veins in the skin, often giving the face, throat and upper part of the chest a flushed appearance and a warm sensation. This is due to the fact that morphine releases histamine. This is also the reason for the itching and perspiration often seen in opiate users.
The effects mentioned so far do not explain the mood changes which occur with opiateuse, and even less, the phenomenon of 'addiction'. These are dependent on the influence of opiates on the largest cell complex which is strewn with opiate receptors, the limbic system and the nucleus accumbens. In this way,opiates cause euphoria, but lessen negative stimuli such as pain and distress, leading to emotional indifference often combined with inhibition of the sexual functions. The effect is comparable to that on pain: the signal is not removed, but the emotions linked to it are.
Chronic use of all opiates leads to a definate tolerance and a strong physical dependence. The relative severity of the abstinence syndrome is in general related to the duration of efficacy, leading to the paradoxical situation in which the abstinence syndrome of heroin, although occurring extremely quickly, is nonetheless less extreme than that of methadone.
The various opiates.
Opium is smoked, eaten and injected. The duration of efficacy is long (at least 36 hours) and the abstinence sydrome, although slow to occur because of the long period of effectiveness, is severe and longlasting compared to for instance, heroin.
Another means of preparation is that of the 'compote', a boiled down extract of the whole poppy plant, which is very popular in Poland and other former East Block countries, where poppy cultivation takes place on a large scale for the seeds.
Morphine is, as we have seen, the most important opiate derivative of opium. The morphine content of opium is between 5 and 15 %. The period of effectiveness of morphine is 4 - 6 hours. The usual therapeutic dose is 5 - 15 mg.
, In 1832 another opiate was isolated in opium: codeine, which is used mostly as a cough remedy.
When morphine is heated in waterfree conditions with acetic acid anhydrides, two acetic acid molecules combine with the morphine molecule and make diacetylmorphine. This substance was synthesized for the first time in 1898 by Dresser and was put on the market commercially by the firm, Bayer under the name by which the substance is still known: heroin, together with another new product, 'aspirin'.
Heroin is more fat soluble than morphine and thus passes the bloodbrain barrier more quickly. It therefore works more quickly, but for a shorter time: 34 hours. The usual therapeutic dose is 4 mg. With an equal analgesic effect, it has even less influence on the consciousness than morphine.
Heroin was used in the first place for patients with the then incurable tuberculosis. The patient died anyway, but without coughing and pain, and with a hefty dose, in euphoria. The second indication was....'to combat' morphine addiction. Just as morphine was originally used 'to combat' opium addiction, heroin was now used to combat morphine addiction. The result speaks for itself.
Nonmedical use of heroin was primarily an American problem. After the lifting of the alcohol prohibition, both the mafia which had in the meantime come into being and the exopponents of alcohol threw themselves upon 'drugs'. Particularly after World War II, morphine was extracted from Turkish opium in small southern European (Marseilles!) laboratories, and was then turned into heroin. This was then smuggled into the US: the 'French connection'. It was only late in the sixties that some of this production leaked away to the European population (Note 16). At the same time, opium began to leak away from the Chinese communities to the indiginous Europeans. They also went over to injecting the opium.
In 1972, the opium supply suddenly dried up. To begin with, it was thought that this had to do with effective police action, but it soon became clear that it had more to do with developments in SouthEast Asia. With active support from the CIA, heroin production was heavily increased and then dumped on the West European market. Within no time, all the former opium users had become heroin users.
With equal speed, heroin became the vogue amongst the socalled 'speed freaks', users of amphetamines and metamphetamines (Pervitine), who combatted the overstimulation caused by these substances with heroin, often taken in a 'shot'.
Heroin use then spread steadily to newer groups, particularly the various immigrant groups which had begun to populate Europe in the mean time.
After the end of the Vietnam war, the flow of SouthEast Asian heroin, the socalled 'brown sugar' dwindled quite quickly, although it was compensated for by a growing supply of SouthWest Asian heroin, originating in Pakistan and Afghanistan. A connection with the war in Afghanistan seems clear. In any case, the fierce competition between these two heroin sources led to a sharp fall in heroin prices. The provision of methadone has, in Holland, also certainly had an influence on prices.
Heroin is the first example of a semisynthetic opiate: changes in the morphine molecule are brought about chemically. Many other semisynthetic opiates followed, such as Methadone, a synthetic opiate, which was synthesized by German chemists at I.G. Farben at the end of World War II while researching spasmolytica. The narcoticanalgesic effect was discovered only after the war, this not being expected as the chemical structure differed so much from the known opiates.
The story about methadone being produced to take the place of morphine, which was said to be in short supply as a result of the obstruction of opium imports in favor of war activities, is untrue. The name Methadone was given by the Council on Drugs of the American Medical Association in 1947.
It differs from morphine and heroin in that it is broken down in another way, works well taken orally, and works for longer. It is said to be effective for 24 hours, although there are many indications that 20 hours may be nearer to the truth. In any case, efficacy varies considerably in duration from individual to individual.
Methadone administration.
Methadone is used as a replacement for heroin, in treatments aimed at either abstinence (reduction cure) or maintanance. For the medicinal treatment of abstinence syndrome, it appears to work no less well than Librium. NOTE 17
The idea of substitution and withdrawal is not particularly modern: a royal decree given by the second King of Thailand in 1809 suggested a course of selftreatment for opium dependence consisting of gradually cutting down the daily dosage until complete abstinence was reached. NOTE 18
Supplying methadone to heroin addicts was discovered by Dole and Nyswander, and was based on a somaticmedical theory: heroin use apparently brings about a metabolic disorder which makes further use of heroin or other opiates a necessity. By supplying methadone, which should correct this disorder and block the euphoric effect of heroin, patients should be able to function normally. NOTE 19 NOTE 20 The results of treatment appeared to be impressive. NOTE 21 NOTE 22 NOTE 23 At last there was a medical treatment for heroin addiction aimed at improved functioning: methadone maintenance.
The rationale for this treatment was and still is that supplying methadone would reduce the criminality associated with heroin use and would enable heroin addicts to lead a socially productive life, or, as it was put in a recent Dutch methadone pamphlet, Methadone is intended to enable the user to function without the use of illegal substances and without the related risks such as criminality, infectious diseases etc.' NOTE 24
As has already been stated, people's expectations regarding the results of supplying methadone appeared to be surpassed by what actually happenned: Gearing's evaluation of Dole's program NOTE 22 NOTE 23 showed that 50 80 % of patients had returned to a 'socially productive' life.
Yet Trimbos NOTE 25 who, already in 1971, visited Dole & Nyswander's program in New York, had his doubts. Where addiction was the result of character defects, methadone treatment only masked the defects, and as such was only a cheap show of success. Dole and Nyswander rejected premorbid sociopathic behavior as the cause of the addiction and Trimbos felt more or less obliged to follow them in this opinion, impressed as he was by the data he was given, but he remained wary and missed 'attention to the sociogenesis of the addiction disease'. His doubt can be read between the lines. Besides, once the opiate receptors had been discovered by Kosterlitz in 1972, it was of course, easier to avoid the either/or position prominent at that time.
In 1972, Maddux and Bowdon NOTE 26 fired some sharp criticism at the methadone treatment. They state that 1) the replacement of heroin by methadone in itself is already regarded as a success, 2) the reduction of criminal behavior is not clearly demonstrated in the published data, 3) the stated number of those in work averages only 15% higher than in drugfree treatment programs, and it is in no way clear that this 15 is due to the methadone. They regarded the reports about the success of the methadone treatment both ambiguous and exaggerated.
Preble, NOTE 27 whose own field observations gave him a distinctly less rosy picture than the results presented by Gearing, showed that Gearing's figures originated from the programs and these results in turn were based on unverified information given by the patients themselves. Gearing had himself not checked how his patients were faring, and his findings, as such, were totally unreliable. Preble himself described the behavior of the socalled socially productive people as follows: "They pick up their methadone, get their welfare check and pay their rent, buy pills and drink booze", an image with which we in the Netherlands have become all too familiar in the eighties. Only 14% of the population investigated by him used the methadone as it was intended, that is, remained free from all other drugs, did not engage in criminal activity and were in work. Besides, Haddox and Jacobson NOTE 28 indicated as early as in 1972 that it is possible to predict whether or not the maintenance treatment will work, with the aid of psychological tests.
So far, we have the history of methadone administration in the US. In the Netherlands, we see a completely different development. Here too, in the late sixties, opium addicts were treated with methadone for the first time and this was extended to a regular program after the introduction of heroin in 1972, but in contrast to the American programs, it was a reduction program here, in which far lower doses were kept to. However, recidivism after completion of the cure turned out to be the rule rather than the exception, followed by readmittance to the program for another cure. This practice grew into a sort of zigzag pattern; no maintenance, but a series of gradual reductions of dosage alternated with sudden increases.
In 1977, the Stichting Kontakt Sentra from the HUK started up with a major 'low threshold' maintenance program for its clients, whom they already knew from years of observation.
'With this assistance, we hope to bring about circumstances in which the role of heroin in the addict's life will have less emphasis (humanizing), that the rhythms of the user's life will become more regular, whereby access for further help will be increased and all kinds of activities (work, living outside of Amsterdam, group outings, etc.) can be realized much more easily . NOTE 29 ' With the so called low threshold character of this program, it should be noted that this refers primarily to the demands made within the program itself of those taking part, while entrance to the program was extremely difficult, aimed as it was at only very problematic addicts.
The goals of this program were very different from both the maintenance programs in the U.S. and the gradual outpatient detoxification/resocialization.
The belief that a junkie taking methadone will become a respectable citizen a la Dole & Nyswander, which still appears to survive in some circles, was in any case, thrown overboard.
The treatment concept disappeared. Others suggested a new, equally medically oriented vision to take its place. It was based on the concept of 'selfmedication'. In this vision, the supply of methadone is seen as a remedy given according to phychiatric indication: methadone as an antipsychotic and/or major tranquilizer.
This is best illustrated in the principle of 'basic' administration, formulated by W.G. Mulder in the early eighties on epidemiological grounds. This became the guideline for Amsterdam's methadone policy. Come, swallow and go, was the motto, combined at the most with elementary medical care. There was no place for other sorts of intervention aimed at rehabilitation within this system, and if clients wanted that, they had to look for it elsewhere. Amsterdam politicians have translated this (most unmedically) as the principle of 'Do ut des', I give in order that you shall give, with which is meant that the junkie gets his methadone in order that he will behave more sociably.
The mistake in this thinking is that the junkie on the one hand wishes to alter his behavior, while it has long been clear that heroin use gives meaning to the life of the addict NOTE 30 NOTE 31 , and on the other hand, the expectation that the proffered methadone can come close to the desired heroin. But the junkie who finds his way to a methadone program does not do it out of desire for treatment which aims at a change in behavior which he himself is also striving for, but as a pharmacological emergency intervention, to which he goes only when he is unable to survive independently. He does not want to stop with the dope, but he cannot be it for a longer or shorter period continue the way he has been carrying on: he is too old, too tired, to weak. NOTE 32 NOTE 33
Both the outpatient detoxification programs and the programs aimed at only the most extremely problematic drug users have in common that the supply is seen as only part of a total treatment aimed at rehabilitation. It is ironic to note that precisely what they are attempting to prevent with the low threshold 'basic' administration is taking place here in the Netherlands. But in the abovementioned article by Preble NOTE 30 and in an article by Soloway NOTE 32 , the effects of large, open methadone maintenance programs were already made clear in the seventies. Under these circumstances, the administration of methadone is merely a palliative. Just about all that was described at that time took place in the Netherlands in the eighties. He who will not learn when told, must learn by experience.
Giving methadone to addicts in all its various forms is still regarded as medical treatment, pharmacological intervention. Practice gives us another view. 'Basic' supply without a structured treatment program aimed at rehabilitation is not treatment at all.
Who continues to prescribe a medicine if it turns out not to work? There is little doubt possible about the fact that in a considerable proportion of methadone clients, methadone use does not lead to the significant social rehabilitation described by Dole & Nyswander.
On top of this, practice teaches us that supplying methadone entails 'for life' in the majority of patients. Not that there would be anything against this if it worked, but we see that it doesnot.
There is even a poorly thougtout pronouncement by a Dutch court that methadone is an addict's right. Each Dutch citizen of course has a right to medical facilities and these rights are laid out in the statutes for medical insurance. Medical insurance companies however, quite rightly refuse to pay for (the more or less 'basic') methadone programs, as methadone supply in the Netherlands is not usually a medical treatment. Methadone is provided by the government as a subsidized stimulant for every junkie.
Another model worth considering is combining supply with reform: methadone will be given as long and only as long as the patient stays on the straight and narrow. This would be a rational application of the 'Do ut des' principle, according to the American model of Dole & Nyswander, but subject to strict individual control. If someone fails on a program such as this, that is, gets into trouble with the police, he would have to wait for, for instance, 2 years before being allowed to start again. We are no longer dealing with medical treatment, but with functional behavior seen from the eyes of the upholders of order. The number of participants in such a program would not be much more than 5% of the present methadone clients.
In 1984, De Mug, (a Dutch paper for the jobless) NOTE 34 suggested that methadone be used purely as a medicine, thus treatmentoriented. We can safely assume that no more than 10% of the present number of patients would be eligible for this. In any case, giving methadone remains a possibility as part of an integrated treatment in which concrete goals and checkpoints have to be reached at a certain time. In the case of failure, the methadone might be quickly phased out.
'Basic' administration as a medical function is now being extended as bait, particularly in relation to AIDS, but then one is tying people to a longlasting pharmacological chain from which it is more difficult to get free than it is from heroin. It would be more logical with regard to AIDS, just to legalise heroin. Taking part in a methadone program does not appear in any way whatever to increase one's sense of responsibility and that is after all the main point where the HIV problem is concerned, not to mention resocialization.
Of course, methadone or morphine can be administered intravenously as a palliative treatment (for instance to addicted AIDS patients), but this is also possible with heroin.
The reader could be forgiven for thinking that this writer is opposed to methadone. That, however is not the case. He is just as much or as little opposed to methadone as to heroin or other drugs. Even though the original goal of administering methadone, that of rendering property crime unnecessary for the procuring of opiates, has not been reached to a significant extent with large numbers of addicts, the legal, medicalized provision of methadone has not only had a positive effect on the heroin market, but also on the impact of the AIDS epidemic.
No, this writer is against the hypocrisy in the foundations of methadone administration. The inevitable conclusion is that methadone administration in the Netherlands has in many cases, no medical or scientific aim, and is thus in conflict with the opium law, and is in fact maintained in order to undermine the law.
This is, of course, a serious matter. This writer has pleaded before for the abolition of the opium law NOTE 35, as the penal approach to the misuse of substances has not proved itself to be an unmixed success. Addiction is bad enough without turning the addict into a criminal. This observation compels us to ask whether we are indeed happy with the 'uneasy concensus' between doctors and the keepers of order who, for the greater part, decide what form the present methadone policy will take. It's fine to pick holes in the opium law, but abolish it then, or if that isn't possible due to 'international' consequences, at least admit that you are circumventing the law by, for instance, removing all the rigmarole around handing out the drugs: give each junkie a credit card to take to the local pharmacy for his desired portion instead of to a drug 'helper'. And let him pay for his credit card noone gets his kicks for nothing.
Opiate antagonists
The problem of addiction led to the search for substances with the painkilling effects of morphine but without its addictive effect. The first result was nalorphine, an opiate antagonist which can thus be used for the treatment of an acute overdose of opiates. This material blocks the opiate receptors, so that they cannot be stimulated by opiates. It must be applied with care: an overdose provokes an acute withdrawal syndrome among opiate addicts! All the same, nalorphine also proved to have some effect as an opiate agonist and was thus subjected to further research.
The next substance was a full antagonist: naloxon. This genuine antagonist has virtually no effect except after the use of opiates. Soon afterwards naltrexon was developed. This substance has the same effects as naloxon, but it is more suitable for oral application and is effective for a longer period. These substances completely block the effect of opiate agonists without any appreciable sideeffects. The substance is virtually nontoxic. If naltrexon is administered in sufficient doses (120 mg 3x week), the effects of heroin are completely countered. The pharmacological basis for naltrexon therapy which is proposed in the US and is also being considered in the Netherlands is thus beyond discussion. Since the blockade is competitive, high doses of heroin can overcome the naltrexon. However, this seems pointless: such large amounts of heroin are expensive, and it is easier to take naltrexon once than simply to carry on using heroin. There is therefore no point in naltrexon maintenance for those who do not want to live a clean life, but it is useful for those who do. Naltrexon can protect this group against unforeseen situations in which they might give in to sudden temptation (the return of an old friend from the scene, etc.).

Two-thirds of all heroin users chase the dragon. Yet little is known about it. Does the aluminium foil give off harmful fumes? What happens when the dope is cut with something else? And how much dope do you lose when you chase the dragon? The views of some users a doctor and a laboratory analyst.
It started off as a rage in Hong Kong early in the fifties. It got called chui lung, which means chasing the dragon. Because the spiralling smoke looks like a dragon' s tail. The fumes are inhaled through a small tube. In Holland smoking heroin only started to become popular in the seventies when the main supply of heroin came from China. That's why chasing the dragon has become known as 'chinezen' (Chinesing) in Holland. A good two-thirds of all users in Amsterdam take their heroin Chinese-style.
Carlos has been chasing the dragon right from the start, fifteen years ago now. "I came to Holland from Surinam and was taken in by an aunt. I spent a lot of time with my two cousins. Smoking grass, and hanging around the streets a bit. One of my cousins had a chromium-plated convertible Chevrolet in perfect condition. Those were the days, man! All the dope you wanted, beautiful women, and evenings dancing in Marcanti. Both my cousins were users already, so I asked them once could I try it, chasing the dragon. One thing led to another.
I've never done anything else but smoke it, earlier on in cigarettes as well, but usually on foil. There aren't that many Surinamers who shoot up, I only know a few. It's much more common with Dutch kids. They often begin with the needle."
Joop began with the needle. "I was six-teen when I had my first shot. After two weeks I could fix without any help. I didn't think of smoking the dope. I didn't even smoke cigarettes. I only started doing that a few years later. " For ten years Joop shot up. First only heroin, later on coke and cocktails as well. A year and a half ago he stopped injecting and started chasing the dragon instead. "It was my veins, you see. I didn't have easy veins to start with, but in the end I didn't have any left that I could use. I always shed a lot of blood, and of course I went on trying, because you want your hit no matter what it costs. Certainly if you're on coke. I stopped with coke first, because that just got completely out of control. A couple of months later I stopped shooting up. I hadn' t had that blissful flashout for years anyway. Chasing the dragon suits me better. It's also better for my health."
Extra filter
Chasing the dragon is less of a health risk than mainlining. When you shoot up everything goes straight into the blood-stream: not just the heroin, but also any-thing it' s been cut with and any other toxic substances. Using a swab of cotton wool can give you a degree of protection against most of these substances, but a lot can still come through the needle itself and so go straight into the blood. In any case all soluble elements and anything that is extremely small, bacteria for in-stance. It's the shit and bacteria that comes with the shot that cause all the misery that many mainliners experience: abscesses,
the shakes, blood poisoning. Chasing the dragon means that no toxic substances can get into the blood-stream directly. You have an extra filter, your lungs. The inhaled heroin fumes enter the lungs via the air passages. And the pulmonary blood vessels absorb the heroin. Only after that does it enter the bloodstream. Many people ask: even if those toxic substances don't all enter my bloodstream directly, aren't they just as damaging for my lungs?
"I've never come across a lung problem that was only caused by smoking heroin," says doctor Krish Kanhai. He has been working for eight years for the Health Authority as a doctor specialized indrugs and he is quite clear on possible risks for the lungs of smoking heroin. "Don't get me wrong, it isn't healthy. Smoking is always bad for the air passages, whether it is tobacco, marihuana or heroin." He stashes his pack of cigarettes and continues. "The fumes of some substances that are present in the dope can cause the air passages to contract. The air passages can also become irritated. All this together can lead to bronchial complaints. A lot of people get an increased production of phlegm. Heroin suppresses the tickle in the throat, which means you don't cough up all that phlegm. This causes it to accumulate lower down and this can lead to pneumonia. But that' s not something you get easily. Only if someone already has poor health or has been sleeping out-side in the cold or doesn't look after him-self properly, do you get problems like that. This doesn't come from chasing the dragon but from other reasons." It' s a different matter when someone already has lung problems. " Chasing the dragon can be dangerous for someone who has a tendency to bronchial asthma. It can give you a severe attack. I know two people who choked to death during an attack like that." But in general Kanhai does not regard chasing the dragon as a health risk: "In any case it is safer than shooting up. "
White chocolate
The foil is to someone who smokes what the works are to the mainliner. Only it is less complicated. A syringe must have a sterile wrapping, a good rubber and a sharp needle. With foil you don't have to be so careful. As long as the foil is metal and can be heated well and gradually. In practise a roll of kitchen foil is the best. The thicker the better. "When I don't have any foil," Carlos says, "I buy it on the street from hustlers who sell it or from a fish stall. And sometimes I buy chocolate. Some brands of chocolate also have good silver paper. Galac's white chocolate, for instance, the kind without the rice. It's often bought just for the wrapping. That's why in places where people chase the dragon you often see bars of chocolate Iying around. In case of need I sometimes use the foil in cigarette packs. First you have to burn off the paper and the foil is very thin. You can burn holes in it easy as anything. But when in need it will do."
First of all the piece of aluminium foil is made completely smooth. For instance with a piece of toilet paper. Then the dope can move freely round the foil. Some people heat the foil a liffle before they put the heroin on it. This burns up the thin plastic layer on the foil. The plastic is there to prevent the foil from rusting and to make it easier to roll out. With its normal household use you don't have any problems with the plastic but inhaling burnt plastic is bad for your health. "Of course smoking plastic is harmful," says doctor Kanhai. "The same goes for the fumes of aluminium that has been burned through being heated too long. It is better let the plastic burn off the foil first."
A taste of the grave
Chasing the dragon has another plus point. People who smoke heroin can more easily tell the quality of their dope. Joop: "When you hase the dragon you can first try out a little bit and taste it to test whether you've got good stuff. When you mainline it's different. You dissolve the dope, fix a shot and all at once you've got it in you."
Good quality dope melts when you heat it, turns into a light brown oil and can move easily around the foil. Carlos: "If it leaves a trail of little black specks, then you're okay." When the oil sets it turns into a resin-like substance. The resin should be hard as glass and transparent. "It's also a good tip to take the drop of resin from the foil and hold it up to the light. If it comes easily off the foil without crumbling and if you can't see any impurities when you hold it up, then you can assume your dope is good." You can also tell good dope from the taste you get when you smoke it. " Some people say it' s a taste that's bitter and sour at the same time. Other people say it tastes like putty. I think it has an unpleasant taste, a taste of the grave, but at least it' s one you can recognize out of a thousand," Carlos says. "But the main test is whether you get stoned from it or not. And you can tell this with the first puff. If you don't feel any-thing with the first puff, then it's shit."
Skill
Not all kinds of heroin lend themselves easily to chasing the dragon. You'd think that Chinese heroin from the Far East would be the best to smoke. Because of the name and because Chinese heroin dominated the market at the time when chasing the dragon became popular in Holland. But it turns out that 'Turkish' heroin from countries like Pakistan and Afghanistan is more suitable. This kind of heroin is prepared in a different way from the Chinese. It only dissolves after some lemon juice or ascorbic acid has been added, but it makes an easiersmoke.
H. Hauser from the forensic laboratory in Rijswijk came to the same conclusions. He did research into chasing the dragon. He placed different sorts of heroin and heroin cut with other substances on some foil and heated the samples with a lighter. In two seconds the temperature rose to six hundred degrees centigrade. Hauser collected the fumes and checked out the amount of heroin contained in each sample. The percentage of heroin in the fumes turned out to depend mainly on the kind: there is three times as much heroin in the fumes from Turkish heroin as there is in the Chinese. What the dope was cut with also proved to have a great influence on the quantity of heroin in the fumes. Cutting it with caffeine produces positive results: it increases the ease with which one can smoke the heroin. Caffeine already starts evaporating at a hundred degrees and one might say that it takes everything else with it. After heating a mixture of Turkish heroin and caffeine sometimes as much as three-quarters of the heroin remains in the fumes. All other ingredients (paracetamol, mannitol, strychnine, lactose and glucose) that the heroin is cut with diminish the pro-portion of heroin in the smoke. When heroin is cut with noscapine (which is a by-product of careless preparation of the heroin) the percentage of heroin in the fumes is a third less.
In theory it's possible that someone will get more stoned from chasing the dragon with weak heroin that has been cut correctly than with much stronger heroin that has been cut with bad ingredients. The way in which it is heated is also very important for the effect. Heating it too quickly means more gets lost. The more skilful you are in smoking it, the more you get.
To each his style
"You have to learn to chase the dragon," says Joop. "I've noticed that I have a completely different style from Carlos who has been smoking much longer than me. You see, he smokes in a way that's very painstaking. He makes a tiny flame and succeeds in sucking in almost all the smoke. He can spend hours over one packet. I' m different. I' m much greedier and so I lose a lot of smoke. That' s why I've lined the inside of my tube with aluminium foil. After I've smoked there's always a bit of red deposit left. That is pure dope that I haven't absorbed. I keep this foil for the next morning. First of all I heat up the deposit till it turns into a drop and then I smoke it.
"It suits me perfectly. It's more healthy and I get more pleasure out of my dope. I take longer over a packet and I still feel good. I am also more sociable. Smoking together is much more sociable than shooting up together, isn't it? You also feel more inclined to give some away. Compare it with someone who sits sociably at the bar and treats everyone to a round. That's totally different from someone sitting in the toilet on the sly and tossing back the contents of a hip-flask. I don't think of myself as a junkie any more, but as a user."
